About the role
This is a challenging and rewarding opportunity for you to make a positive change in the lives of young people in Out of Home Care in Perth Metro.
We are looking for a Case Worker to join our team who run Transitional High Needs placements for LWB in Perth metro. This is a specialised care model for clients with complex needs who have often experienced significant trauma and may display behaviours of concern including aggression, self-harm, drug and alcohol use, absconding and sexualised behaviour. This means the young person will be living in a house where we provide 24/7 care with Residential Support Workers providing 1:1 or 1:2 support to the young person.
The purpose of the Case Worker role is to develop and implement case or individual plans for young people in THN care ensuring we provide a safe and caring environment to assist the young person to meet their goals and re-engage in education and their community.
This is a full-time permanent position, based from our Leederville office (with some remote WFH or work from residential care houses each week). This role come with a Tool-of-Trade vehicle, laptop and mobile phone.

Successful candidates will be required to clear probity checks including National Criminal History Record Check, Department of Communities Check and Working with Children Check.
