About the Role

Actively support our aged care clients with a disability, chronic medical conditions or mental health who access the Queensland Community Support Scheme services. Our clients are based in Ipswich you will support multiple clients per day between split shifts across the hours of 8.30am- 5pm Monday-Sunday in a casual capacity.

Key Responsibilities

  • Assist clients to improve their independence in according to their identified needs and goals
  • Operate from a wellness framework, building on the client’s strengths and focusing support in line with the client’s goals and aspirations as a path to improved quality of life
  • Establish a rapport and collaborate effectively with clients and their family/carer to achieve positive outcomes
  • Maintain and update professional skills by actively participating in personal, professional, service development and training opportunities.
  • Support the client to develop their skills and enhance their independence with showering, bathing, eating, grooming and any personal care needs as identified in their QCSS Plan
  • Support client to develop their skills and enhance their independence in making meaningful connections and accessing the community through activities

Skills & Experience

  • An unrestricted Australian Driver’s Licence
  • Certificate III in Individual Support (Ageing, Disability or similar)
  • A current First Aid Certificate
  • Excellent organisational skills, hygiene practices and problem-solving abilities
  • Strong communication skills with the ability to build and maintain positive relationships

Successful candidates will be required to clear all state-based probity checks; including but not limited to a National Criminal History Record Check and NDIS Worker Screening Check.
