About the role
We have an opportunity for an experienced and enthusiastic professional who can work independently and with resilience. This role is a mixture of work from home, and out of our office based in Hurstville office, you will be managing a case load of young people in family-based care and supported independent living. Due to the nature of the work, a willingness to participate in a on call rotating roster is imperative.
As a Case Manager you will develop and implement case plans for children and young people in care. Working closely with young people, carers, and their families, you will progress plans for permanency as agreed in the case plans.

Skills & Experience
Successful candidates will be required to clear probity checks including National Criminal History Record Check and Working with Children Check (where relevant to the role).
