About the role

We have a new opportunity for an experienced Case Worker to provide supervision to disability Host Carers who are caring for clients accessing Independent Living Options (ILO) Host Care services.

In this role you will develop and implement care plans for clients and coach Host Carers using the CARE model. The objective of the position is to ensure that every Host Carer has an opportunity to develop in their caring role, has adequate support and has the necessary skills to meet the carer competencies set out in LWB’s Disability Assessment Manual. This role operates in partnership with the NDIS and alongside the Department of Communities, families, office of the public advocate, carers and other stakeholders.
  
This is a full-time position which can be based from Perth or Bunbury. If you are Perth based, you would need to travel to Bunbury around 3 days per week to fulfil this role. You will receive a laptop, iPhone and Tool of Trade vehicle. 
  
Key Responsibilities

  • Undertake disability focused casework practice in partnership with the NDIS, client’s families, carers and other stakeholders. This includes undertaking a range of activities:
    • Assessment of the individual, their support network including carer families
    • Case planning
    • Implementation, monitoring and review individuals care plan and goals
  • In a disability and CARE informed way, work in partnership with carers to:
    • Understand the impact of the individual’s disability and at times the trauma they have experienced;
    • Provide high quality, safe care for individuals that is focused on supporting positive life outcomes.  This will include building strong relationships, supporting reflective practice and providing regular supervision and reviews;
    • Apply the CARE model in day to day care with individuals to enable them to learn life skills/build on their existing living skills, form relationships and gain resilience;
    • Build and strengthen carer skills on how to include individuals in decision making and support them to fulfil their potential;
    • Plan and manage the transition of the individual into a placement, facilitating introductory visits and supporting the individual, the carer and their family and the individual’s family.  Ensuring the carers have all relevant information prior to the individual being placed;
    • Support carer to attend to all medical and therapy related goals by attending medical appointments, liaising with stakeholders, requesting assessments to determine level of need.
    • Where relevant, perform case management functions as prescribed in specific contracts or funding agreements.
    • Participate in negotiation between LWB, the NDIS and all other relevant parties to ensure that each individual is provided with care that meets their needs.

*Must participate in on-call roster as required*

Skills & Experience

  • Recognised Social Work Qualifications with eligibility for membership of the AASW and/or a Bachelor of Psychology or other relevant qualification.
  • Experience developing quality case plans or individual plans 
  • Demonstrated experience working autonomously with multiple clients simultaneously
  • Experience working with individuals, families or groups from a culturally appropriate perspective
  • High level communication, influencing, liaison, negotiation and mediation skills
  • Current unrestricted driver licence
  • Senior First Aid certificate
  • NDIS Screening Check (via DoT)

Successful candidates will be required to clear probity checks including National Criminal History Record Check, Department of Communities Check, Working with Children Check and NDIS Screening Check
