About the role
We have a new opportunity for an experienced Case Worker to provide supervision to disability Host Carers who are caring for clients accessing Independent Living Options (ILO) Host Care services.
In this role you will develop and implement care plans for clients and coach Host Carers using the CARE model. The objective of the position is to ensure that every Host Carer has an opportunity to develop in their caring role, has adequate support and has the necessary skills to meet the carer competencies set out in LWBs Disability Assessment Manual. This role operates in partnership with the NDIS and alongside the Department of Communities, families, office of the public advocate, carers and other stakeholders.
This is a full-time position based in our Leederville office. You will get a laptop, iPhone and Tool of Trade vehicle. Please note, due to the operational requirements and the nature of our services, it may be required for the incumbent to work outside the normal span of hours.
